Prior Authorization Is Where Good Care Gets Stuck
Your provider ordered the MRI. The patient is ready. Then the request sits in a payer queue, comes back asking for one more chart note, or gets denied because the wrong CPT code was on the form. Meanwhile the schedule has a hole in it and the patient is calling your front desk.
The same problems show up in almost every practice we work with:
Nobody knows the rules changed
Payers add codes to their authorization lists every quarter, and benefit managers like eviCore and Carelon have their own criteria on top of that.
Thin medical necessity packets
The request goes out without the conservative treatment history, imaging results or failed medications the payer wants to see.
Mismatched codes
The CPT or J code authorized doesn’t match what was billed, so the claim denies even though an approval exists.
Expired or exhausted approvals
Therapy visits run past the approved count, or a surgery slips past the authorization date.
No time for follow-up
Pending requests go unchecked for days, and peer-to-peer windows close before anyone schedules the call.
Every one of those turns into a delayed treatment, an unhappy patient or a claim you can’t collect.
A Prior Authorization Team That Also Bills the Claim
Most PA vendors stop at the approval number. We also code and bill, so we make sure the approval matches what will actually be billed: same CPT, same units, same dates, same rendering provider and location.
Ready for Medicare WISeR
Arizona is a WISeR state. We know which Original Medicare services now need review, how to submit, and where the UTN goes on the claim.
AHCCCS plan know-how
Each AHCCCS Complete Care plan has its own PA list, portal and fax rules. We work them every day.
Benefit managers covered
eviCore, Carelon, Evolent and the other vendors that review imaging, therapy, cardiology and oncology for Arizona plans.
Strong packets the first time
We pull the notes, imaging, labs and treatment history payers ask for, matched to criteria like InterQual, MCG or the payer’s own policy.
Peer-to-peer and appeals included
When a request is denied, we prep your provider for the call and write the appeal if it’s still needed.
Status you can see
Every request, its payer, its stage and its expiration date in one daily list your front desk can check.

Medicare Prior Authorization Services for 2026: What Changed in Arizona
Two federal changes hit Arizona practices this year. One adds prior authorization to Original Medicare. The other puts deadlines on how fast many plans must answer.
The WISeR Model Brings Prior Authorization to Original Medicare
Arizona is one of six states in CMS’s Wasteful and Inappropriate Service Reduction (WISeR) Model, along with New Jersey, Ohio, Oklahoma, Texas and Washington. The model runs from January 2026 through December 31, 2031. For a defined list of services, such as skin and tissue substitutes, electrical nerve stimulator implants and knee arthroscopy for knee osteoarthritis, Original Medicare claims now go through either prior authorization or pre-payment medical review.
In Arizona, the WISeR technology participant is Zyter TruCare, and Noridian (Jurisdiction F) remains the Medicare contractor. Approved requests come with a Unique Tracking Number (UTN) that must appear on the claim. Medicare Advantage patients are not part of WISeR; their plans already have their own PA rules.
How we help: we check every Original Medicare order against the WISeR code list, submit requests with the right documentation, track the UTN and make sure it lands on the claim. If a request isn’t affirmed, we help you decide whether to resubmit, request MAC review or move to pre-payment review.
The CMS Prior Authorization Final Rule (CMS-0057-F)
Since January 1, 2026, Medicare Advantage plans, Medicaid and CHIP programs and their managed care plans, and Marketplace plans must decide expedited requests within 72 hours and standard requests within 7 calendar days, and give a specific reason when they deny. By January 1, 2027, those payers must offer electronic prior authorization APIs. That means AHCCCS Complete Care plans and Medicare Advantage plans in Arizona now run on a clock, and we hold them to it.
Services Original Medicare Already Required Auth For
Certain hospital outpatient department services (such as blepharoplasty, botulinum toxin injections, rhinoplasty, vein ablation and some spinal procedures), certain DMEPOS items and repetitive scheduled non-emergent ambulance transport.
Prior Authorization With Every Payer Your Patients Use
The plan on the card isn’t always the company that approves the service. We know who reviews what, and where each request needs to go.
| Payer group | Where requests go | What to watch |
|---|---|---|
| Original Medicare | WISeR portal or Noridian for WISeR services; Noridian for OPD and DMEPOS programs | UTN on the claim, and the pre-payment review option |
| Medicare Advantage | Plan portals and Availity, often routed to a benefit manager | Plan PA lists, 72-hour and 7-day decision clocks |
| AHCCCS Fee-for-Service | AHCCCS Online prior authorization | Requirements differ from the managed care plans |
| AHCCCS Complete Care plans | Mercy Care, Banner University Family Care, Arizona Complete Health, UnitedHealthcare Community Plan and others | Separate PA lists and portals per plan, behavioral health rules |
| Commercial | BCBSAZ, UnitedHealthcare, Aetna, Cigna, Humana and Banner|Aetna portals | Quarterly PA list updates and site-of-service rules |
| Benefit managers | eviCore, Carelon, Evolent and similar vendors | Imaging, cardiology, therapy, oncology and sleep criteria |
| Pharmacy benefit | CoverMyMeds and PBM portals | Step therapy, quantity limits and formulary exceptions |
| Military and veterans | TRICARE West and VA Community Care | Referral and authorization before every episode of care |

| Sleep medicine | In-lab polysomnography and home sleep tests |
| Inpatient admissions | Notification, admission review and continued stay |
A Note on Emergency Services
Emergency care does not need prior authorization under federal rules for Medicare Advantage, Medicaid and most commercial plans. What payers often do require is timely notification of an inpatient admission and authorization for post-stabilization care. We file those notices so a true emergency doesn’t become a technical denial.
Our Prior Authorization Process, From Order to Approval
Order received
We pick up new orders and scheduled procedures from your EHR or a shared work queue each day.
Requirement check
We confirm whether the payer, or its benefit manager, requires authorization for that code, place of service and diagnosis.
Packet built
We gather the documentation the payer’s criteria call for and flag anything missing to your clinical staff.
Request submitted
Standard or expedited, through the right portal, with the CPT, ICD-10 and units that will appear on the claim.
Follow-up
We track every pending request and respond to information requests the same day.
Decision recorded
Approval numbers, dates, units and expiration go into your system and onto the appointment.
Denial handled
If the payer says no, we set up the peer-to-peer, prep your provider and file the appeal if needed.
Turnaround
| Request | When we submit |
|---|---|
| Routine orders | Within 1 business day of receiving a complete order |
| Urgent or expedited orders | Same business day |
| Requests for more information | Same business day |
| Status checks on pending requests | Every 24 to 48 hours until a decision |
Payer decision times then follow each plan’s rules, including the 72-hour and 7-day CMS windows where they apply.
Denied Prior Authorizations: Peer-to-Peer and Appeals
A denial on a prior authorization request is often a documentation problem, not a final answer. Many denials can be reversed with the right conversation or the right records.
Read the reason
Payers covered by the CMS rule must now give a specific reason. We match it to the criteria the payer used.
Fix and resubmit
Missing records or a coding mismatch can often be corrected with a new request.
Peer-to-peer prep
We schedule the call inside the payer’s window and give your provider a one-page summary of the case, the criteria and the points to make.
Written appeal
If the decision stands, we draft the appeal with clinical evidence and track it through each level, including external review where available.
Root cause
Repeat denials by payer or code are reported back so the next request goes in stronger.
When a claim is still denied after the service, our denial management team takes it from there.
